Dependent Verification Procedures

Employees who wish to cover their eligible dependents on the Union County CEBCO group health, dental, and/or vision insurance program(s) must provide suitable verification of dependent eligibility before coverage will be granted. Documents that establish verification must be submitted to Human Resources within 30 days of the requested insurance coverage effective date. Failure to provide the required documents will result in denial of the dependent coverage request, in accordance with group insurance plan requirements.
