UCATS Forms

Listed below are the categories of residents that are eligible for free transportation.  Please complete the applicable forms and submit to UCATS electronically, mail (PO Box 389, Marysville OH 43040), or in-person at 940 London Ave, Suite 1800, Marysville OH 43040.

• If you are a Senior (age 60 or more), complete number 1 and 3.
• If you are a Medicaid recipient, complete number 1 and 2.
• If you are a client of the Union County Board of Developmental Disabilities, complete 1 and 4. 
• If you have a Disability, complete number 1 and 5 below. A disability is defined by the Ohio Department of Transportation as an individual with a physical or mental impairment that substantially limits one or more major life activities of such individual; a record of such an impairment; or being regarded as having such an impairment.

2. Release of Information for Union County Department of Job and Family Services (purpose is to determine eligibility for grant funding)
3. Release of Information for Union County Senior Services (purpose is to determine eligibility for grant funding)
   
4. Release of Information for Union County Board of Developmental Disabilities (purpose is to determine eligibility for grant funding)
